MD Perform uses an at-home testing kit that measures key nutrient markers in your body. You collect a simple sample at home, mail it back using our prepaid label, and receive your results straight to your inbox.
Yes. Our testing method is completely safe, painless, and non-invasive. No needles, no doctor's office visits—just a simple at-home urine collection that takes less than 5 minutes.
We test over 125 key nutrients that directly impact athletic performance: magnesium, B vitamins (B12, folate, B6), zinc, antioxidants, amino acids, neurotransmitters, and several other critical markers. Each nutrient we test can have a profound impact on energy, recovery, and performance.
After your sample reaches the lab, average processing time is 21 days. You'll receive an email notification when your results and targeted supplement recommendations are ready to view.
Your testing kit includes everything you need: collection materials with easy-to-follow instructions, prepaid return shipping label, comprehensive lab analysis, personalized results report, and targeted supplement recommendations. Beyond that, you’ll receive exclusive access to MD Perform’s Membership Portal with educational resources covering everything from how to interpret your test results, to mental mindset and recovery strategies.
About Supplements & Recommendations
Yes. Based on your test results, you receive specific supplement recommendations—including dosages. You’ll find out what your body actually needs, taking the guesswork out of buying supplements.
No. You can purchase your recommended supplements from wherever you prefer. However, as an MD Perform Member you get exclusive access to our curated selection of pharmaceutical-grade and sports-certified supplements through our partner dispensary at a 5% discount.
All recommendations are based on evidence-based medicine and are appropriate for youth athletes. Families should review any supplement decisions with their own licensed healthcare professional, especially for athletes with medical conditions or medications.
That's fine! Your test results will show whether your current supplements are working or if you need to adjust. Many athletes discover they're wasting money on supplements they don't need or missing key nutrients entirely.
About Pricing & Plans
- Single Test Kit: $695 (10% off for add-ons)
Testing is currently self-pay. However, you can submit your receipt to your insurance company or HSA/FSA for possible reimbursement. We do not guarantee insurance or HSA/FSA reimbursement.
We recommend testing 2-3 times per year—typically before, during, and after your athlete’s season. This allows you to track progress, adjust your plan as your athlete's body changes, and ensure optimal nutrient levels year-round.
No. You only pay when you order a test. There are no hidden fees, subscriptions, or auto-renewals.
About the Process
From ordering to receiving your targeted supplement recommendations:
- Order your kit (ships within 5-10 business days)
- Kit arrives at your home
- Collect sample and ship back (5 minutes)
- Receive results and targeted supplement recommendations to address deficiencies (average of 21 days after lab receives your sample)
Every test purchase includes access to comprehensive support. You may also be able to schedule a follow-up consultation with Dr. Biondich for an additional fee if you want to discuss your results in detail.
Yes! We offer a 10% discount for households with more than one athlete. Each athlete receives their own personalized results, targeted supplement recommendations, and access to MD Perform’s Membership Portal.
MD Perform is specifically designed for young athletes ages 12-18. However, athletes of any age can benefit from MD Perform’s precision nutrient testing.

Dr. Amy Biondich is a board-certified emergency medicine physician (MD, MS, ABAARM, FACEP) board-certified and specializing in functional and regenerative medicine. She has over 15 years of experience in emergency medicine and has worked as a Medical Director for Ultra Endurance Marathons caring for elite athletes around the globe.
No. While elite athletes benefit greatly from precision testing, MD Perform is designed for ANY athlete who trains seriously and wants to optimize their performance—whether they're playing high school sports, club teams, or training for the next level.
MD Perform is based in Eagle, Idaho, but we serve athletes nationwide through our at-home testing program.
